At Mississippi Periodontics, Implants & Esthetics, we are grateful for the opportunity to assist you in achieving a healthy and functional smile that you'll love.

Your initial exam will last approximately one hour. Dr. Howard will review your medical and dental history and perform a thorough examination of your teeth and gums. Because we know your time is important, we try to stay on schedule to minimize waiting.

In order to make the most of your visit, we ask that you fill out the below patient form before your visit or arrive 15 minutes early to your appointment to fill out.

Things to bring on your first visit.

Please assist us at the time of your first visit by providing the following information:

​

  • Your referral slip (if you have one), health history and registration forms

  • Your completed new patient form linked above

  • Any X-­rays from your referring dentist (if given), including any panoramic X-­rays taken within the past six months

  • A list of medications you are currently taking

  • Your dental insurance card, if insured

​

Please arrive approximately 15 minutes prior to your consultation so that we can verify any information. A parent or guardian must accompany patients under 18 to this consultation appointment.
